Medicare / Coverage Conflicts

Medicare billing conflict

If you receive a bill and Medicare should be primary, call the Benefits Coordination and Recovery Center (BCRC) at 1-855-798-2627 or visit cms.gov.

“My provider says I’m not primary...”

HRA is not considered primary insurance. Refer providers back to Medicare if applicable.

Insurance won’t pay out

This is likely because they believe Medicare should cover it first. Call the Benefits Coordination and Recovery Center (BCRC) at 1-855-798-2627.

Admin & Dependents

Attestation Form

You must confirm that your non-HRA insurance meets minimum coverage. This is required and time-sensitive.

Adding a Dependent

Enter information directly in the HRA Portal.
• For babies: No additional paperwork needed.
• For marriage: Upload marriage certificate.

Report a Death

Submit a death certificate. If there is an eligible dependent, their account may be created. Call 1-800-251-4107 (option 2).

HRA FAQs

This is not like a 401(k), you cannot cash it out. You also cannot roll this over to another HRA.
To file or submit a claim, the easiest way is to fill out one of the forms below and upload to our portal here: HRA Portal.

If you’re not yet registered, please give us a call at 1-866-408-7158.

For one-off claims, use the short claim form here: Short Claim Form

If we will be paying your monthly insurance premium on a recurring basis, use the RECURRING claim form here: Recurring Claim Form

Note: You must attach supporting documentation for each expense such as an itemized bill from the provider or an Explanation of Benefits (EOB) from your insurance company.

You may submit the form to us one of four ways:

- By uploading to our secure portal here: HRA Portal
- By mailing to the USW HRA Fund, 1101 Kermit Dr., Suite 800, Nashville, TN 37217
- By faxing to 1-615-333-5797 (make sure to include the 1 at the beginning of the number)
As of 2024, we no longer do bi-monthly payments. Now we pay every week.

If we receive the claim by EOD Thursday, we will process no later than 3pm on Friday to receive payment for the following week.
No, you do not need to pay the provider before you submit a claim. The only ones that are required to be paid first before submitting a claim are for braces and hearing aids. If you have more questions, please give us a call at 1-866-408-7158.
If you’re an employer, please give us a call at 1-800-251-4107.

If you’re an employee and your employer has negotiated the HRA, you will receive a packet that you need to complete and return to us. Reimbursements from your account will not begin until forms have been completed and returned. If you have any more questions, please give us a call at 1-800-251-4107.
When we deny claims, we send a letter with a denial code and also post it to our secure portal here: HRA Portal. (If you’re not yet registered, please give us a call at 1-866-408-7158.)

To find out what that denial code means, please find the Denial Code PDF here: Denial Code PDF.
In order to submit or update your direct deposit information, you must contact the Fund Office at 1-800-251-4107.
Note: You do not need a separate form to change your address.

Before you fill out the form, be sure you know your bank’s routing number and your account number. If you have a checking account, this information can be found on the bottom of a check (see image).

Example check

**If you are using a reloadable debit card, the account number and routing number should be provided from the company who issues the cards. (The account number is not the number on the face of the card).

If you are unsure of the routing number and/or account number, call your bank to confirm.

Important Dates
Updated Direct Deposit Forms must be received on or before the 15th of the month prior to your next payment to be processed on time.
To change your address, contact the Fund Office at 1-800-251-4107.

Summary Plan Description (SPD)
The HRA is funded solely by employer contributions required by your collective bargaining agreement. Federal law currently prohibits employee contributions.
The HRA allows your employer an opportunity to offer a benefit that you can use to cover the rising cost of health care. Your employer can deduct contributions made to the HRA on their income tax return, and your employer is not required to pay FICA taxes on contributions made on your behalf.
